Direct Donation Form

If you would like to donate by Cash or Check and receive a receipt, please print the below form and submit the form along with your donation to either Ms. Willett  or to Board members  during any General or Executive meeting.  Your form will be signed and returned to you as a receipt record of your donation.  Your tax deductible donation goes directly to helping fund student music activities, instruments, and maintenance.  Without your help, this music program will not be possible.
